 | Age 18 or older. |
| Have a current FL-2 completed, dated, and signed by physician within 30
days of admission, indicating DOMICILIARY HOME placement. |
| Have Tuberculin test record or chest x-ray with results in accordance with
minimum standards. |
| Have no contagious or communicable diseases. |
| Provide facility with any past psychological , medical evaluation and
social history. |
| Have available medications as prescribed on FL-2 or prescriptions for
these medications. |
| No resident will be admitted in need of treatment for acute mental
illness. |
| No resident will be admitted in need of professional nursing care under
continuous medical supervision. |
